The liver is one of the most important organs in the human body, responsible for filtering toxins, producing bile for digestion, and storing essential nutrients. A swollen liver can affect other parts of the body and produce pain or discomfort in unexpected areas.

1. Upper Right Abdomen

The liver is located in the upper right side of the abdominal cavity. When it swells, it puts pressure on surrounding tissues and organs, leading to pain or a feeling of fullness in this area. This is often the most direct and noticeable sign. The pain may be dull or sharp, and it may worsen after eating or lying on the right side.

2. Right Shoulder

Swelling of the liver can irritate the diaphragm, which shares nerve connections with the shoulder. This can cause referred pain—a situation where the discomfort is felt in a different area than where the problem actually is. Many people with liver inflammation report an aching or stabbing pain in the right shoulder.

3. Back and Rib Cage Area

Because the liver is located just below the rib cage, swelling can lead to discomfort or tenderness in the lower ribs on the right side or even radiating into the mid-back area. This pain can be constant or occur intermittently, depending on how severe the swelling is.

4. Upper Middle Abdomen

A swollen liver can also extend toward the center of the upper abdomen, causing pressure or pain in the epigastric region—just below the breastbone. This may be mistaken for stomach pain or indigestion.

5. General Abdominal Discomfort and Bloating

In more advanced stages of liver inflammation or damage, fluid may accumulate in the abdomen (ascites), causing bloating, heaviness, and generalized discomfort. This is not only painful but also a clear sign that liver function is severely compromised.

Other Symptoms to Watch:

Yellowing of the skin and eyes (jaundice)

Nausea or vomiting

Loss of appetite

Dark urine

Pale stool

Pain associated with a swollen liver isn’t always obvious or severe, but recognizing where and how it manifests can be critical for early diagnosis. If you feel consistent pain in the upper right abdomen, shoulder, or rib area—especially when paired with other symptoms—seek medical attention immediately.
